Seafarer Medical Fitness – Health, Safety, and Responsibility at Sea
Working aboard a seagoing vessel requires not only professional competence but also physical and psychological resilience. Regardless of rank or area of operation, all crew members must be capable of performing their duties reliably under often challenging conditions—over extended periods, in varying climate zones, and far from immediate medical care.
For this reason, holding a valid Medical Certificate of Fitness for Sea Service is a legal requirement for professional activity at sea. It ensures that seafarers are medically fit to fulfill their responsibilities safely and without posing a risk to themselves or others. The legal basis for this certification is provided by the Maritime Labour Act (SeeArbG) and the Maritime Medicine Ordinance (MariMedV).
Why is medical fitness for sea service so important?
A medical emergency at sea cannot always be promptly treated by a physician on shore. The Seafarer Medical Fitness Examination is therefore an essential component of onboard safety protocols. It ensures that all crew members are capable of carrying out their duties responsibly and without undue health risk—to themselves and to others. The medical standards are internationally harmonized and aligned with the guidelines of the International Labour Organization (ILO) and the International Maritime Organization (IMO).
Who is authorized to perform the examination?
Seafarer medical fitness examinations may only be carried out by physicians who are specifically authorized for this purpose. This authorization is issued by the Maritime Medical Service of BG Verkehr. Authorized physicians* possess specialized qualifications and experience in maritime health and are subject to regular review.
Authorized German Social Accident Insurance for Transport conducts medical fitness assessments for sea service—professionally, confidentially, and in full compliance with all legal requirements.

Examination process
The assessment begins with a personal consultation to record your medical history (anamnesis). This is followed by a series of standardized medical evaluations, including:
Assessment of visual and auditory acuity
Measurement of blood pressure, pulse, weight, and urinalysis
Physical and neurological examination
Depending on pre-existing conditions: additional diagnostic tests (e.g., ECG, laboratory values, X-rays, etc.)
The assessment is tailored to the intended service branch (e.g., nautical  or technical department). In some cases, specific conditions or limitations may be noted in the certificate.

In cases of specific medical conditions or health risks, the validity may be shortened on an individual basis.

If an individual is assessed as unfit for sea service, there is the option to appeal the decision through the Appeals Committee of BG Verkehr. We are happy to support you through this process.

Please prepare for the examination by bringing the following documents:

  • A valid identity card or passport
  • If applicable: your current eyeglasses, contact lenses, or hearing aids
  • If you have chronic illnesses: recent medical reports or a medication plan
  • If available: previous medical certificates for sea service

  • If applicable: a letter of cost coverage from the shipping company
  • Parental or guardian consent for minors

For minors under 18 and employees of companies affiliated with BG Verkehr, the examination is generally free of charge. In other cases, we will inform you transparently in advance about any applicable fees.
